In Regards to the Texas Education Freedom Account (TEFA) Program

As we seek to obtain clarification from government officials on the necessary documentation needed to support signature by the Audiologist on the Disability Certification form to be submitted with your application, we are currently needing the following:

  1. Current (within the last year) audiological evaluation
  2. Education assessments from a diagnostician or teacher statements indicating accommodation or validation of impact on learning.

If your child has an individualized education program (IEP) on file with the Texas Education Agency by the end of the application period, the Comptroller’s Disability Certification Form is not needed to apply for the TEFA program. You will only need your child’s Texas Unique Student Identification number (UID) and the numerical instructional code found on the IEP.

It is not necessary to certify more than one disability to establish the child’s priority for admission to the program.
